Snoring is a noisy sound produced when breathing during sleep, caused by uvular (the fleshy downward hanging conical body at the top of your throat) and pharyngeal (the tube connecting the mouth and nasal passages) vibrations. The noise of snoring may exceed the 90dB (a smoke alarm emits 100dB..). Snoring wakes up the snoring person, their family and leads to sleep deprivation and chronic fatigue. A more serious condition related to snoring is sleep apnoea.

Prior to undergoing any procedure, we will conduct careful diagnosis to identify whether your snoring is related to your nose or throat.

The first diagnostic stage involves all necessary examinations including taking a detailed Ear, Nose and Throat history of the patient, ENT examination with endoscopic assessment of the nasal cavities, nasopharynx, oral cavity, larynx and the throat. Moreover, an ENT specialist will check where the breathing obstruction is located.

The second stage of the diagnostic assessment involves examinations performed when the patient is sleeping. It involves night-long monitoring, recording and analysis of the patient’s statistics.

Stages of the patient’s sleep are assessed along with their heart rate, number of breaths per minute and oxygenation level.

We make use of a diagnostic procedure involving examination with an apnograph – which allows us to identify any narrowing in the respiratory tract, which in turn facilitates referring patients for surgery or for non-surgical treatment.

Nasal reconstructive surgery:

For those patients where snoring is due to abnormalities in their nose or upper airways,opening of the obstructed nasal passages aids in the relief of snoring and sleep apnea. The procedure may be to reduce the mucus membrane in the nose or may require reduction of the nasal septum.
The first stage of treatment involves restoration of normal patency of the nasal cavities by means of corrective surgery of the nasal septum and reduction of the size of hypertrophic nasal conchae. Nasal conchae size reduction can be performed surgically by means of resection or bloodless ablation. Ablation (or radio ablation) is a procedure in which the internal section of the nose is shrunk with no damage to the external surface. The procedure is performed at the clinic, under local anaesthesia.

Approximately 30% of patients require some form of nasal surgery prior to undergoing UPP.

Uvulopalatopharyngoplasty (UPPP):

The most common cause of snoring relates to the throat and specifically, the abnormal anatomy and function of the soft palate and the uvula. In this procedure the respiratory space is increased by excision of excessive mucosa of the palate along with a part of the uvula. In some cases, palatine tonsils may be removed as well. This procedure can be performed with the use of classical surgical methods or with the use of ‘coblation’, with a low-temperature device (plasma knife) that utilises plasma formed by radio waves. In this treatment high frequency radio waves create small scars on the palate in order to elevate it. Similar scars are used to shrink the base of the tongue if it is hypertrophic (enlarged). Thus a larger area for air to pass through is created.

In surgical patients this method is used to reduce healing time and pain. Post-operative complications occur rarely.

Sleep Apnoea

Sleep apnoea is a symptom of obstruction of the respiratory tract during sleep. The level of sound emitted by snoring persons varies, reaching as much as 90 dB in extreme cases. Apart from being intrusive for the snoring person’s environment, this condition can be dangerous to the patient as well, as it results from a temporary oxygen deficiency.

The most progressed form of snoring is the obstructive sleep disorder syndrome. It is an illness, which causes an individual to pause breathing for up to 10 seconds, accompanied by a decrease in saturation (arterial blood oxygenation) by 4% or more, with such pauses occurring at least five times per one hour of sleep.

Patients with sleep apnoea syndrome often complain of morning fatigue with headaches, reduced ability to concentrate and memory disorders. They often fall asleep during the day in various circumstances (on their way to work, while driving a car or amidst a conversation). Apart from snoring, excessive daytime drowsiness is the most prevalent symptom of the sleep apnoea syndrome.

If sleep apnoea syndrome is confirmed on the basis of the diagnostics, the patient is referred for further treatment.
